I want to know if the address is require for state inspection for cars?

-I just took my car for state inspection, i gave my credit card to the guy, and he took my credit card and went inside my car to do the inspection without returning the credit card. now when i was leaving he asked me to write my name and address, and i asked him why he said he need to put it in the system. Do you think the address is requiered for state inspection ? he also told me that I have to change the windshield wiper or my car wont pass. there was just a little tear on it, i guess he did it just to sell he's stuff for $10.
